Hello, is it possible for a Moderator to change my forum username? I've already changed it once, but months ago. Can I have a refresh, and change it to 'xFatalityx'? Thank you.

Post what you just wrote in the link above ^. I'd advise to read what Sean has posted for more information. Once he has changed your name, he'll delete your thread. Hope this helps you and resolves your issue
