Have you ever been banned? (if yes, why?): I was banned once. That was when I first joined for accidentally griefing my coop partner. I took some potions out of a chest and used them!
Why do you think you should be a mod?: I would like to be a mod because; 1. I like helping people out, 2. I would like to give beginners the logs if they were griefed. If they were griefed I would replace what was stolen or broken with my own resources.
How long can you be active on the server every day?: I can be active about 7-9 hours. I have to go to school ( 8:25am to 3:25pm. It takes my 1 hour to get home. Then I do my homework which normally takes 30 minutes.)
Do you have any past experience as a moderator?: Yes, On one server I was a moderator. Then the owner of the server thought I was a good moderator, Which lead to me being an admin1

Ok :3. btw I do go online a lot and help eg; I do free gods and repairs! And help people with there islands, If I do get to know the person well, I give them stuff like Diamonds or good diamond pickaxes etc...

 

No support. I have never seen you online or on the forums. You need to add a lot more detail, space out the questions, bold only the questions, and add some color.
