Hello, I hope the tips I am about give you help improve your Application:

    1. Make your answers into sentences. This helps the readers and Admins understand you are fully capable of writing.
    2. Bold your questions and space them out from each other. This helps distinguish between which section the readers and Admins are in.
    3. The question "Why do you think you should be a mod?" give the readers and Admins more information such as traits, personalities, skills, etc. :) This helps get to know you better.
    4. When the question asked, "What qualifications do you have?" instead of putting none put stuff like Are you a quick typist? Are you very social? Stuff like that helps improve the Application.
    5. For the question, "How long can you be active on the server everyday?" make a schedule for Monday-Sunday. Not only does this give the readers and Admins more information on the Application but it also gives more information about your daily life. :)
    6. The last question, "Do you have any past experience as a moderator?" put down how long were you / have you been a Moderator on the servers. Put whether or not it was large / small server. This gives the Admins more information on whether or not you need training with the commands. :)

Hopefully these tips help you improve your Application! Good Luck! :D

P.S.- To Edit your Application, go to the very bottom of where you have written and it will say "Edit". Click it! ^.^

    Banned once?
Sorry, but I believe you've been banned 5-6 times.

Also you need to make the questions stand out from the answers or it's all muddled and more complex to read.
And you need a lot more detail. You need 5-6 lines per question (Apart from the simple ones, IE: Minecraft name) :)
