Have you tried doing /spawn or going home with /home? I am online right now if you require assistance as I can tpa you somewhere.
If you are falling forever it just is because you don't die at spawn. As said above do /home or /spawn to go back. If you cannot connect to the server then you will need to contact a mod. This is best done using our discord. If you don't know the link to the discord I will provide you with it.
