Why do you think you should be a mod? I play on this server a lot (all my time playing Minecraft is on this server) and whenever someone in chat asks a question, I am usually the first to reply. I try to stop people from breaking the rules, and whenever someone does, I politely PM them telling them their mistake. I have helped quite a few people start their island, and I try to be as nice and polite. I have a lot of knowledge of Minecraft. As this is one of the most popular Minecraft servers, with the stress of people on the server's resources, some parts of the game stop working. (Like the double-block mining glitch) I can help overcome these issues and make the server more enjoyable for all players.
How long can you be active on the server every day? About 4 hours on weekdays, and a bit more on Saturday. (I have commitments on Sunday)
Do you have any past experience as a moderator? I own a Minecraft server that me and a friend have completely configured and built ourselves. We planned to release it to the public, but it is whitelisted now and you have to apply to join. As said a lot, I have lots of knowledge of how Minecraft works and I like coding with Java in my spare time.

~ add some more color into it it looks a bit dull or add lighter colors ( i'm not recommending yellow)

Visually, your application looks fine. Don't feel the need to embellish it any more than it already is. Instead, you should just elaborate more on the details...try focusing on your qualifications. You could also add some info about yourself because it doesn't seem like a lot of people know who you are.

Other than that, no support (yet). You seem like a mature, responsible player, but I can't pass a solid judgement quite yet. Familiarize yourself with the community more before you try to apply as a moderator. Good luck.

 

First off, I would add some more light colours, but that's just me. Secondly, I think you need to add more detail, specifically to the 'Why do you think you should be a mod?' part. Also, you could add a 'Traits' part, just a thought.
