Please not that this is by a high ranking donor, not a moderator. Mods can and probably will fix any incorrect information.
    1. Create Proper Looking Titles
When you have a good title, mods will look at it quicker. The best ban appeal format is:
[Username] Reason (Ex: Griefing, Spamming, Bridging) [Mod who banned you]
With an Example Being: [RandomPerson] Swearing [Noobcrew]

If you use a different format like "Plz Unban me! :(" or "unban me naw plox!!!11", the process will take longer, because you need that mod who banned you to approve/decline the appeal.

    2. Use the Right Format
If you do the wrong format, you will get a reply saying to use a format, and if changed, you will have an auto-reject. If that happens, then you are in some deep you-know-what. The format will give enough information for other admins to help, along with evidence the mod who banned you. This is the correct format with an example:
    1. Server name (.net / .org ): Skyblock.net
    2. In-game name: ImNotARealUser
    3. Full exact Ban Message: Banned, appeal at the forums. ~NoobCrew~
    4. Did you read the rules ? No
    5. Why do YOU think you were banned? I was banned for swearing
    6. Was this an incorrect ban? No
    7. Were you ever banned before on SkyBlock? If so, what for? Yes, for bridging between 100 islands.
    8. Why do you think you should be unbanned? I realized the error of what I did, and I promise not to swear again.
    This is the incorrect format:
    Worst Version:
    0mg, l3t m3 back in plox!!!11 i d1d n0th1n wr0ng plox!!!1 ill g1v 9001 d011ars!!!11111111

Best Version, but still wont be accepted:
I was banned for swearing in the server. It was a provoked swearing from ImAlsoNotARealPerson because he threatened to kill me. I promise not to swear again.

    3. NEVER Tell a Lie
If you lie to a mod, there is a chance that your ban appeal will be denied because they may think you will lie just to get back in, and do everything they did wrong again. Mods KNOW when you lie.

    4. If a Mod asked a Question you do not understand, Ask for clarification
Mods are always able to clarify anything they ask. If they ask for an answer in a term you do not understand, or if a rule is unclear, ask them to clarify, and they or another mod will help give a rewording.
Ex: Before: We are always wary of what people say. Did you think you could get away with cussing?
After: We are very aware of what people say. Did you think you could swear and not get banned?

    5. Do not make a Ban Appeal just because you Can't enter
Even Mojang AB knows that Minecraft and Minecraft servers glitch. There is a point where the Minecraft servers that control versions and skins could be offline, or where there is a coding glitch in bukkit. Make sure you ask a mod if the server is bugged or you really got banned (and did not get a ban message.)

    6. Never tag people
If you tag them, the mods will make you one of the last one to have your appeal checked. Mods can have up to 10 appeals going at once, along with much more, including messages, conversations, feedback, moderator applications, and other threads.

More may be added later. Feel free to reply below any ideas to get added.

Looks great. Hopefully people take the time to read this before submitting a ban appeal. Maybe it should be stickied to the appeals section?

Think you meant to say "mod" there.

Thanks for finding that error! XD
We will need to ask a mod, and I wont do that because I'm the writing, I need someone else to do it.

Also, if you can change the title from "effecting" to "effective" that would be more understandable.

Not sure if it's possible, an admin/mod would have to probably.
