    You didn't put any effort into this so I don't truly think you want to be a moderator. Read the template, look over more popular applications and edit yours. There's many mistakes in it and whether you didn't know there was a template to fill out or you just didn't care, both ways show that you don't really care to become a moderator and you just aren't ready. Just because you want to become a moderator doesn't mean it'll be handed to you on a silver platter, you have to work for it.

    No support please follow template to have a better chance at getting mod also put your location in title e.g:
    [USA] your name's (trial) mod application form
