I don't see how yours is more "professional" and anyhow what's the point of having a "professional" form when adressing 15 year olds, but okay. It's a variant.

Well, "korrekt" is a bit how you'd talk to a mate for example. I'm not saying yours is bad, but it's more like you'd talk to a friend than you have to fill out a formula.
