Make it easier to read, also make it longer!

 

Personally, I think it is fairly easy to read, but I do think it could be a bit longer

 

I'd like to see the questions in your application bolded.
Generally it smoothes the layout of your application a lot.

Apart from that, I'd try to fill out the application with a little more personal information about you as a human being, rather than your dedication to the server. Tell us what you do and what you enjoy, it'll get you far.
