Add a discord ban appeal template/format

So when somebody is appearing a discord ban, they kind of have to make up their own template. Some people follow the ban appeal format, but it just doesn’t fit most of the questions. So I suggest making a discord specific template to avoid confusion.

Support, this would make things easier to sort out and spot the discord bans
